6 Week S.H.I.P Program

The S.H.I.P program is designed to provide six (6) one-on-one sessions (conducted over phone or via Zoom) to help you alone or the caregivers/support systems of geriatric patients traverse the dangerous waters of our healthcare system via the S.H.I.P. model which entails:

• Safe and effective use of all medication therapy for the senior loved one and/or caregiver.
• Healthy lifestyle promotion through evidenced based counseling/coaching.
• Independent living for the senior loved one by resolving recurrent health care and lifestyle barriers.
• Provision of resources that promote senior and caregiver well-being.
