This service is designed to smooth the transition from hospital to home by reconciling all medications and ensuring that there are no glaring medication-related problems that could increase the risk of re-hospitalization.  This entails looking at duplicate therapy, drug-drug interactions, drug-disease interactions, appropriate dosing based on patient specific data, as well as assessing the complexity of a drug regimen based on the senior’s independence and support systems.  I will outreach patient’s health system to try to determine ideal formulary medication to keep patient costs down.  This program is also designed to outline a prioritized and detailed agenda list for the patient’s follow-up visit with their primary care physician.  This agenda list includes: clinical concerns I may have, patient identified concerns, and caregiver concerns.  Preparation for physician appointments is vital, in ensuring all relevant aspects of patient care are addressed in the patient’s follow-up appointments.
