Deeper Dive Medication Review

The Deeper Dive Medication Review is a 2-session medication management program for the senior patient who is on multiple medications, and they or their loved ones, feel their medication therapies are suboptimal given their chronic conditions.  I offer expertise in creating a medication review plan that is tailored to the senior, while working in conjunction with their primary care physician.  Topics covered in the medication review may also include the interplay between drug therapy and lifestyle modifications, and I am present to help patients be accountable for factors such as (but not limited to) smoking cessation, weight reduction, evidence-based supplement reviews, and chronic disease management strategy implementation.  Periodic follow-up is done to determine the success of clinical interventions, and if the senior needs a more comprehensive program to achieve goal directed health outcomes.
