Bronze Review (one time charge of $200 per person)

The Bronze Review is a minimum 2-session medication audit (with up to 4 month access) that is ideal for the patient on 4 or less medications, and who have 3 or fewer medical conditions that are well controlled.  This program is ideal for patients who want to: 1) get a thorough review of all their medications and health issues to ensure there are no red flags on the horizon, 2) those who want to get off medications and 3) Patients who want an overall second opinion on their medication therapies, 4) Those who have a single health condition or problem that negatively impacts their life.  An example of someone who would benefit from this program is Susan:

Background

Susan is a 75-year-old female who takes 4 prescription medications, which includes one for cholesterol, another for blood pressure, a sleeping pill, and a thyroid medication.  Susan is very active and enjoys walking, traveling, cooking and participating in church activities.  Susan has been on the same medications for years and often wonders if she still needs to be on them.  Physically, aside from minor aches and pains (which she attributes to getting older) and allergy flares, she has no major complaints.  Mentally, she has some concern about recent memory lapses and privately worries it’s a cause for concern.

The Silver Med Review minimum 6-month program is for the patient on 5 to 10 medications, who have medical conditions such as depression, diabetes, heart failure, osteoporosis, asthma, chronic obstructive pulmonary disease (COPD), chronic pain, recurrent urinary tract infections, coronary artery disease,  and in fact any disease that has resulted in a hospital visit within the last 2 years. This program is ideal for patients who have chronic diseases that are not optimally controlled and whom suspect that their drug therapy is causing multiple side effects that reduce their quality of life.  If you have a chronic health condition, we can help you manage it. Jame's Health journey

James is a 54-year-old diabetic patient who suffers from type 2 diabetes, high cholesterol, obesity, erectile dysfunction, neuropathy, and low back pain.  James takes 7 prescription medications (as well as supplements in which his physician is unaware of).  He longs for the days when he was a high school football star who exemplified vigor, strength and speed.  Now he worries that he won’t be able to walk his daughter down the aisle when she gets married next year.  He and his wife use to travel a lot, but between a stressful job and uncontrolled pain he frets having to do a lot of walking during vacays.  His doctor tells him he has to increase his activity or he would be a candidate for a heart attack.

Ashley's Health Journey

Ashley was born with a congenital heart defect, severe persistent asthma, bone deformities, depression, multiple sclerosis, rheumatoid arthritis, chronic pain, and irritable bowel syndrome.  She is on 14 different medications and has been hospitalized 3 times over the last 8 months.  In spite of her health conditions, Ashley is a successful manager of a tech company.  However, she fears that her health concerns could derail her career trajectory.  She outreaches Deeper Dive Horizons to gain better control of her health, reduce her pill burden, and look for more cost-effective medications.
